Saint Peter the Apostle is offering Sports Physicals on the school campus for the 2015/2016 school year by appointment only!

All you have to do is:

• fill out the sports physical forms found on our website under ‘athletic forms’ or follow this link: https://school.saintpetertheapostle.com/wp-content/uploads/2014/07/SPA-Physical-form-2013.pdf
• $10.00 (Cash or check)
• Turn into Brooke Cameron or the Front Office by Tuesday, May 12th, 2015.

**Please Note: Your child will not be able to sign up until All forms have been completed (pg. 1, 2 and 4 on form above) and fee of $10.00 turned with completed form.**

Who: All student-athletes that attend Saint Peter the Apostle School or Parish.
When: Wednesday, May 13^th, 2015
Where: Saint Peter the Apostle Parish Center – Gym

• Walk-ins will be accepted on a standby basis and are not guaranteed.
• Proceeds will be donated to the SPA athletic program.
